Witherhead, the "Black Plague of Ragni," is a powerful level 15 undead skeleton who resides in her lair in the ancient sewers beneath Ragni, and serves as the final boss in the Decrepit Sewers dungeon (as well as the lowest-level dungeon boss in the game). The dungeon is designed for players of levels 9-18, although it can be unlocked after level 5, and Witherhead can serve as a dangerous foe for lower-levelled players, though the fight with her is relatively straightforward.

Witherhead is first encountered during the Sewers of Ragni quest, in which she is briefly sighted at numerous points in the old sewer tunnel, which is also infested with undead like the Decrepit Sewers are. After unsealing the dungeon, the player may venture inside to destroy Witherhead and the threat she poses to Ragni after being released.

Combat[edit | edit source]

After completing the final combat phase of the Decrepit Sewers dungeon and unlocking the door to the boss room, players must fight and defeat Witherhead to complete the dungeon and claim their rewards. In groups, the dungeon is recommended for players of level 9 upwards, though solo players are recommended to be of at least level 18 before facing the dungeon and Witherhead.

The boss arena is large and circular, with shallow water around the edges, and several level 8 Witherhead Spawn appear around it, attacking in close combat. Witherhead herself attacks with a bow, occasionally also using a dangerous spell similar to the Archer's Arrow Storm to rapidly fire arrows at combatants. She deals a small amount of Thunder damage with each hit, but is also weak to elemental Air; therefore at lower levels, the best available Air or Water equipment is recommended.

At four points around the arena are rows of pressure plates on red clay: these plates activate temporary walls, similar to the ones used to defend against the Flaming Skeletons elsewhere in the dungeon, to provide respite from Witherhead's attacks (her Spawn can still reach you when a wall is up, however). Only two of the four rows of plates is available at any given time; after activating one, a different one will become available. These walls are especially useful in avoiding damage from the Arrow Storm attack.

After Witherhead has been defeated, players will be teleported to the entrance of the dungeon to receive their rewards, including her drops.

Although the history of Witherhead is mostly quite obscure, some evidence exists to suggest her history. In the Secret Discovery A Hero's Origin, located nearby the entrance to the Decrepit Sewers in an old sewer pipe, players may find an old gravestone apparently marking the grave of the hero Bob's birth mother, who died in childbirth in the Ragni sewers during a corrupted siege, a tale told in the books in Ragni's Library; notably, the grave appears to be open and empty, and a trail of black bones leads away from it into the exit tunnel.

Witherhead's dialogue, along with this information, appear to suggest that she is the same mysterious, unnamed woman who gave birth to Bob in the sewers, and died there, apparently later becoming corrupted and rising from the dead to lead the hordes of undead that now inhabit the sewers.
